Ou! r professional staff provides services in the home, in long-term care/skilled nursing and assisted living facilities and in hospitals. The Admissions Nurse must be a RN ~ he/she will be an interdisciplinary team member whose primarty responsibility is to present Compasionate Care Hospice services and philosophy to patients and families in a manner that accesses and admits appropriate patients into hospice care. This position requires traveling within the Clifton territory - this is not an office position. Our ideal candidate will have excellent communication skills (both written and verbal) as well as excellent interpersonal skills. He/she will be self-directed, self-motivated and be able to work well in a team environment. Th! e individual must have a nursing degree and at least three (3)! years of home care and hospice experience.
RN, BSN preferred, NJ license, minimum of three years home health nursing experience, hospice experience preferred. Two years of general nursing experience preferred to include current experience and at least one of the following: hospice, home care, med/surgery or critical care. IV skills and/or certification preferred. Excellent nursing judgment, assessment skills and documentation skills. Excellent oral and written communications and interpersonal skills. Demonstrated clinical competency in applying principles, methods and equipment used in nursing and appropriate specialty. Current CPR certification preferred. Demonstrates sensitivity to the needs of the patients and caregivers Ability to work flexible hours and 'off hours' and to meet weekend, holiday and on-call work; reliable transportation and willing to travel extensively.
