Position Summary:
Creates and coordinates the development and implementation of Individual Habilitation and Service Plans (IHP's and ISPs) for individuals who receive day only services at CVSA. Develops, evaluates, and confirms clinical outcomes of behavior intervention plans to ultimately ensure a functionally-sound behavior plan as needed for persons served. Assists in creating the agendas for family and funder contacts. Coordinates all behavioral and clinical needs of person served based on defined assessment process. Responsible for training, mentoring, and monitoring staff on care delivery. Leads specific IDT processes and is responsible for timely documentation. Advocates on behalf of persons served.

Essential Responsibilities:
1. Creates, coordinates and evaluates the effective development and implementation of Individual Habilitation and Service Plans (IHP's! and ISPs) for persons served to contribute to success in achi! evement of their life goals. These plans will increase individual skill acquisition, and/or meet the specific rehabilitation needs of the person served as well as decrease interfering behaviors:

o In collaboration with the Clinical Director, conducts behavior assessments to include experimental functional assessment/analysis sessions, preference assessments, and treatment assessments in accordance with the IHP/ ISP to identify targeted problem behavior, determine and document behavior goals, identify replacement behaviors, and increase individual skills acquisition of adaptive behavior.

o Creates and maintains master schedule/calendar of day only ISP/IHP's on an ongoing basis for persons served to ensure they are completed and distributed in accordance with Division of Developmental Disabilities (DDD) required time frames and procedures;

o Facilitates comprehensive assessments for day only individuals, identifies goals and service objectives, moni! tors progress via timely reviews in accordance with DDD established reporting requirements. Coordinates the receipt of all IHP/ISP materials from appropriate parties to ensure that all materials are available to implement the ISP/IHP for persons served;

o Coordinates all aspects of the annual IHP/ISP meetings for day only person served and attends meetings in accordance with DDD requirements. Assures that the meeting and the process is person centered.

o In collaboration with the IDT, develops data collection systems to monitor established goals for day only persons served, determines need for improvements or modifications, adjustment of goal criteria, etc.;

o Actively leads the IDT process in conjunction with the IHP/ISP for day only persons served. Attends IDT meetings comprised of circles of support and persons served, program managers, nurses, and staff associates to both provide and obtain information regarding the IHP/ISP, review baseline go! als, identify individual strengths and weaknesses, evaluate progress of! persons served regarding attainment of goals, determine the need for modifications to existing goals, combine goal teaching or rehabilitation and development strategies for best possible outcome, and discuss concerns regarding specific persons served to ensure overall IHP/ISP effectiveness. Coordinates meeting logistics, takes meeting minutes, records action plans, and performs follow-up to facilitate the timely implementation of such plans;

o Ensures the completeness and accuracy of all IHP/ISP files for day only persons served in strict accordance with DDD regulations. Coordinates or completes monthly progress reports, completes quarterly progress reports and six month reviews as required by licensing or funding agencies.

o Specific to all behavior intervention plans, compiles and analyzes data, creates graphic illustrations, acquires consents, and consolidates data for subsequent presentation to, and approval/reapproval of the IDT/TDT, the Behavior Man! agement Committee (BMC), and Human Rights Committee (HRC) as required.

o Interacts with persons served, staff associates, Program management, Division of Developmental Disabilities (DDD) Case Managers, Program Nurses, family members, etc., to develop and communicate level of support needs for day only persons served.

2. Provides ongoing skill-based competency based training and guidance to program associates regarding the behavior, daily care, smart goal skill acquisition, level of support and safety for persons served. Provides feedback to program associates as needed and communicates feedback to the program managers.

3. Establishes effective communications with persons served, parents, guardians and other family members, healthcare personnel, external case managers, operations manager, administration and coworkers across Programs and functions, etc. Operates and communicates in accordance with the Family Handbook.

4. Responsible for ens! uring treatment integrity within programs on a regular basis for the pu! rpose of confirming data accuracy; observes persons served and staff associates and completes integrity and reliability checks to track and document staff performance. Identifies individual and program-wide training needs and coordinates trainings as necessary. Works with the program managers to mentor staff associates.

5. Participates in pre-admission screenings and meetings, to contribute to the evaluation of persons served, discuss transfers and discharges, staffing and clinical needs, etc.

6. Effectively resolves problems that involve people, things, and processes that require general logic and common sense. This may include gathering relevant information, considering alternatives, and drawing logical conclusions based on facts.

7. Completes paperwork in a professional manner, and maintains all documentation as required by the Division of Developmental Disabilities (DDD), Department of Labor (DOL), Division of Vocational Rehabilitation (DVR), ! Joint Commission, and established Bancroft policies and procedures, and as determined by program management; e.g., case management logs, monthly progress reports, assessments, behavioral data sheets, Incident Reports, monthly summaries, Parent/Guardian/Other contact form, etc.

8. Provides direct care to persons served, as needed.

9. Provides on-call clinical support to specific programs on a rotating basis. This includes receiving calls to provide behavioral support, functioning as a QMRP to authorize restraints, and to provide debriefing to staff following restraint use.

10. Projects a positive self-image of professionalism, appearance, confidentiality, courtesy, conduct, honesty, fairness, personal integrity, and a respect for the fundamental rights, dignity, and privacy of others.

11. Performs other related duties as necessary and dependent upon Program supported; e.g., may conduct staff training, etc. Remains flexible and adaptable in! work schedules and work assignments as defined by Program needs.
12. Abides by the Bancroft Code of Ethics, Mission Statement, and Vision Statement in promoting ethical behavior, establishing relationships and providing guidance in decision-making situations. Follows Bancroft Policies and Procedures.

13. Remains current with required training certifications, meets state-regulated licensing and regulatory accreditations, and adheres to mandatory requirements.

15. Willingly and effectively cooperates with Bancroft, The Department of Human Services, The Division of Developmental Disabilities (DDD), and other licensing or state agency or local municipalities in any inspections and investigations, upon request.

16. Maintains a safe and respectful environment, free of abuse, neglect, or exploitation; does not allow weapons, threats, bullying or intimidation. !

Position Summary:
Performs job responsibilities of a Program Associate at designated program sites and serves as a medical liaison with healthcare providers, program management, etc., on behalf of assigned person! (s) served.

Essential Responsibilities:
1. In addit! ion to performing job responsibilities of a Program Associate, serves as a liaison between assigned persons served and their healthcare and insurance providers in advocating the best possible medical treatment for persons served:

o Provides timely medical oversight for assigned person(s) served; e.g., takes vital signs, weighs person(s) served, performs timely insulin checks, ensures fasting for scheduled laboratory tests, etc.;
o Schedules individual medical appointments for person(s) served in accordance with healthcare providers' orders, Interdisciplinary Team (IDT) recommendations, and in compliance with Division of Developmental Disabilities (DDD) regulations and requirements ;
o Prepares assigned person(s) served for all medical appointments with physicians, dentists, and healthcare specialists; e.g., washing, bathing, grooming, appropriate attire, etc.;
o Ensures the accuracy, completeness, and currency of all medical information maintained at th! e program site for assigned person(s) served;
o Prepares for individual medical appointments for assigned person(s) served; e.g., confirms insurance and funding information prior to visit, ensures that all necessary pre-testing has been performed and results received; e.g., laboratory tests, x-rays, special studies, etc.;
o Personally accompanies assigned person(s) served to medical visits or makes arrangements for a proxy; communicates with healthcare providers on behalf of person(s) served, providing medical history, explaining current health status, behaviors and reason for visit, providing details regarding medications and reactions, etc.;
o Performs necessary follow-up post-medical visits; e.g., ensures that drug prescriptions are promptly filled and administered in accordance with providers' orders; ensures adequate inventory of drug prescriptions and other medical treatments for assigned person(s) served; alerts program site staff of any medication change! s; observes and monitors person(s) served post-administration of newly-! prescribed drugs and promptly notifies healthcare providers and program management in the event of an adverse reaction; appropriately updates and files medical information for assigned person(s) served;
o Provides supervision and direct care to assigned person(s) served in the event of illness, surgery, etc., which may vary from short duration to extended periods of time;
o Serves as liaison with healthcare providers, insurance providers, equipment suppliers, etc., in arranging for and acquiring various medical equipment and supplies for assigned person(s) served;
o Serves as an advocate on behalf of assigned person(s) served in a variety of situations.

2. Completes paperwork in a professional manner and maintains all documentation as required (and as applicable for specific program) and as determined by program management for the program site; e.g., daily logs, monthly progress reports, daily goal reports, assessments, daily financial transactions, Med! ical Administration Records, behavioral data sheets, Critical Incident Reports, Parent/Guardian/Other contact form, etc.

3. Maintains effective communications with assigned person(s) served, parents, guardians and other family members, healthcare and insurance providers, own manager and coworkers across programs and functions, Nursing staff, etc. Operates in accordance with the Confirmation of Understanding for assigned person(s) served regarding programs and treatment, healthcare, ethnic and cultural support, physical fitness and healthy living, family visits and involvement, and disagreements and grievances.

4. Provides am support to residential program at designated sites. May be involved in am and pm transport when necessary.

5. Reports to JSC to work within CVSA structured day program at the center on designated day of the week. On these days, first reports to designated residential site and provides am support.

6. May perform other ! duties at designated residential sites as requested by Director/site Ma! nager.

6. Participates in the Interdisciplinary Team (IDT) process in conjunction with the Individual Program Plan (IPP). Follows up and takes necessary action on change in behavioral plans, reports issues in the event plan is problematic, updates records, etc.

7. Follows state-approved emergency procedures and Bancroft-approved crisis intervention techniques at all times.

8. Prepares for, and promptly responds to, emergency situations to ensure the safety and security of the program site and persons served at all times; e.g., participates in fire drills, emergency evacuations, etc. Notifies proper authority in the event of a valid emergency.

9. Serves as a role model to persons served by projecting a positive self-image of professionalism, appearance, confidentiality, courtesy, conduct, honesty, fairness, personal integrity, and a respect for the fundamental rights, dignity, and privacy of others.

10. Where shifts apply at progr! am site, effectively transitions with the staff of the incoming and outgoing shifts to ensure coverage at all times.

11. Ensures the smooth transition of persons served across programs and effectively communicates with the staff members of those programs; e.g., transition from Residential to Vocational, etc.

12. May serve as designated in-charge on a specified shift, in charge of staff and program site activities in absence of Program management.

13. Performs other related duties as assigned or as necessary. Remains flexible and adaptable in work schedules and work assignments as defined by program needs. Will be assigned to work in Vocational Program as needed.

Position Requirements:
Education & Experience:
High School diploma or equivalent required with additional education in a related field desirable. BA in Social Work, Psychology, or a related field preferred. A minimum of two years of experience as a Program Associate or one year as a Senior Program Associate preferably within the Bancroft organization is preferred.

Special Skills:
Demonstrated ability to effectively communicate both verbally and in written form required. Advanced math skills required. Effective interpersonal skills, a strong desire to work with neurologically-challenged individuals required, and the ability to implement crisis intervention techniques required. Operative knowledge of MS Office Word and Excel spreadsheets required. Attainment of CPR, First Aid, Crisis Prevention and Intervention, and Medication certificati! on within first 90 days of employment required. Valid driver's license ! required in incumbent's legal name and current address with no provisional restrictions. Minimum 18 years of age required.

Leadership Responsibilities (if applicable to specific Program site):
When designated as Senior Program Associate, provides supervision to direct care staff assigned to a shift.
