Supervisor, Case Managers


 Overview:
The Supervisor of Case Managers - Treatment Adherence Education position is responsible for overseeing and managing the day-to-day operations of the Case Management Program. This includes managing the Benefits Specialist, Patient Retention Specialist, Case Workers and/or non-medical case management staff This position also provides direct services to persons living with HIV/AIDS and works in close collaboration with other direct service providers, including but not limited to medical and mental health providers. The Supervisor will educate clients about available benefits and assist with the application process and benefits management. When needed, prepares and facilitates relevant benefits appeals and transportation services for clients. Assists clients in obtaining financial, insurance, and other entitlement programs. The supervisor will assist in problem-solving related to clients’ progress toward goals. The Supervisor will monitor the work of the Case Management team through record review to ensure that documentation is appropriate and adequately completed. Ensures and establishes ongoing collaborative relationships with crucial benefits partners.
