KEY REQUIREMENTS

Full unrestricted Licensure, Certification, or Registration required.

Must pass pre-employment examination.

Designated and/or Random Drug Testing required.

Background and/or Security Investigation required.

DUTIES:
This position will require the physician to drive a government vehicle to various home-based locations on a reoccuring basis

. The physician in this assignment serves a geriatric patient population that has multiple chronic medical and psychiatric diagnoses. Care is also p! rovided for a younger population of veteran patients. The most significant aspect of care by the Extended Care Service (ECS) medical staff is timely diagnosis, treatment and follow-up of all the medical and psychiatric assessments, diagnostic workups, consultation and coordination of the interdisciplinary care of patients, giving high priority to their quality of life. Extended Care physicians must be familiar and comply with all Medical Center Memorandums, Extended Care Programs and Extended Care Standard Operating Procedures.

Duties for this postition include but are not limited to:
Serves as Chair of the HBPC Falls Committee which is a model for other departments throughout the medical center and is part of the overall VA NJ- HCS HBPC Falls Committee, which JC cited as a Best Practice.

Screen, admit and discharge patients to ECS

Advance Care Planning

Review documents on hospitalizations, ER visits, falls, and labs

P! rovide medical diagnoses, workups and treatments within the scope of kn! owledge, privileges and patient needs for the patients in ECS. Much of the care involves long term management of various medical problems

Transfer patients to other bed services as necessary, i.e., medicine, psychiatry, neurology and to other VA medical centers as appropriate

Make provisions for, and coordinate care of dying patients and their surrogates including, but not limited to, advance directive, management of pain, psychosocial counseling, spiritual counseling, bereavement counseling, and management of physical problems, giving patients comfort and quality of life the utmost attention

Provide OD and Medical on Call (MOC) coverage

Admit within the scope of Extended Care Service

Diagnose within the scope of ECS

Treat, including prescribing/administering medications, within the scope of the ECS and coexisting conditions.

WORK SCHEDULE:
Full-Time, Monday - Friday, 8:00 AM - 4:30 PM.
QUALIFICATIONS REQUIRED:
BASIC REQUIREMENTS:
U.S. citizenship. Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.

Degree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine.

Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia.

Current, unrestricted driver's license required.

Preferred Experience

: Board certified/eligible in internal medicine and/or family practice is strongly preferred. Board certified/eligible in geriatrics is preferred.

PHYSICAL REQUIREMENTS:
