Assumes system responsibility for initiation, development, implementation and revisions of collaborative care processes relevant to the cardiovascular pati! ent population across the continuum of care. The APN/Care Manager ensures the hospital participation in appropriate national cardiovascular data registries and serves as the pivotal resource to nursing, physicians, case management and outcomes departments. Maximizes clinical performance to achieve "best practice" evidence based benchmarks. Participates with various multi-disciplinary teams to achieve positive fiscal provider outcomes and appropriate clinical documentation for designated patient populations by intervening at key points across the continuum. The APN shall function as a practitioner, educator and leader. The APN is responsible to the Corporate Director of Cardiovascular Services for the development of cardiovascular processes and outcomes monitoring across the continuum of care to achieve clinical excellence for cardiovascular patient populations. (AMI, PCI, HF, Cardiac surgery)
Requirements:
[Minimum education]
The Registered Prof! essional Nurse Practitioner is an Advanced Practice Nurse (APN! ) who has completed course work in pharmacology. The APN has obtained a Master's Degree in Nursing and maintains lecense with the NJ Board of Nursing and follows the standards set forth in the NJ State Nurse Practice Act.

Has at lease 5 years experience in professional nursing. Experience is Cardiovascular nursing is necessary.
